How to Start the Quest
To begin the quest line, you must be at least level 50. However, since some perk cards don't unlock until level 70, it might be worth waiting until you're around level 80 to 100 to fully take advantage of all available perks.
When you log in after the patch, if you’re level 50 or higher, a quest will appear in the top right corner of your screen, prompting you to listen to a distress call on your Pip-Boy radio. This visibility depends on your quest settings being turned on.

Once you’ve listened to the distress call, the quest will direct you to Whitespring Refuge. Simply follow the prompts, speak to the NPCs, and you'll be guided to the next map location, in the top right corner of the map.


Fast travel to the marked location, follow the prompts, and you’ll eventually reach the point where you choose whether to become a Ghoul or not.
If you don't want to turn into a Ghoul, then the quest just remains open in your pip-boy.

There’s lore scattered throughout the new locations, though not much else beyond that.
While you can speedrun the quest in about 10 minutes, I highly recommend taking your time. The NPC writing is excellent, with some witty jokes and well-crafted dialogue. Trust me—good writing isn’t easy, and this quest deserves at least one unrushed playthrough to truly appreciate the effort.
SPECIAL Checks
There is one SPECIAL check, during the first part of the questline.
You will need either 8 Strength or 8 Charisma to select the option.

This outfit comes with the plan to craft it at the armor bench. You will keep both the outfit and the ability to craft it, even if you revert back to being human.

This chem fills your Feral Meter by 75%, making it useful for stabilizing your Feral Meter when you first transform into a ghoul.

There is no plan to craft The Fix
No vendor sells The Fix chem

How to switch between Ghoul and Human
After spending time as a Ghoul, you may want to return to being a Smooth Skin. To do this, you'll need to go through the main menu.
However, if you decide to become a Ghoul again after reverting to human, you'll need to pay 1000 atoms via the Character Menu.
Note for Custom Worlds
If you complete the Ghoul questline on a Custom World, you cannot turn back into a human. The only way to revert is by reimporting or resetting your custom world.
How to Turn Back into a Human
Leave the world you are in and return to the main menu.
Scroll down and select Character.
Locate the Ghoul character you want to revert to human. Ghoul characters are identified by a radiation symbol next to their name.
Select Revert to Humanity at the bottom of the screen.
Confirm your decision by selecting Yes or No when prompted.
How to Become a Ghoul Again
Follow the same steps above to find your character.
Select Become a Ghoul at the bottom of the screen.
Confirm your decision by selecting Yes or No.
If you select Yes, the purchase will be processed, and you will turn back into a Ghoul.
Disguises
Becoming a ghoul will change how the world reacts to you. Some faction NPCs will attack if you approach without a disguise, and certain locations may become inaccessible. Additionally, your Pip-Boy daily quests may be affected while in ghoul form.



Once you turn into a ghoul, a quest will be added to your Pip-Boy, directing you to find and speak to Jaye Vo, who is in the same building where your transformation took place. Jaye Vo will provide you with a full outfit, which you cannot unequip or change. Your other apparel will be greyed out and unavailable, but you can still drop any tradeable outfits or accessories.

The disguise service is free, but removing the disguise will leave you without any equipped underarmor, apparel, or accessories, requiring you to manually re-equip them.

Climbing into Power Armour will not fix the problem. The NPCs will still attack you unless you have the disguise equipped.
To make things easier, they have added an internal and external fast travel option, allowing you to spawn directly inside to speak with Jaye Vo and reducing the number of load screens.

To find Jaye Vo, select the Radiant Hills internal fast travel option on the pop-up screen. After loading in, turn right, and you should see a "Beauty and Spa Salon" sign. Walk toward the two mannequins at the corner; Jaye Vo will be leaning against the room divider nearby or inside the room behind it.
